Ale ta linijka bardzo mnie smuci, z tego co znalazlem w sieci, chyba trzeba znalezc inny dysk :(

No parametr 5 wskazuje, że dysk ma uszkodzone sektory, które zostały zastąpione z puli zapasowej.
Wykonaj kopie danych i dysk do wymiany, ewentualnie na mało ważne dane z dodatkową obserwacją czy wartość nie rośnie.
Aczkolwiek nie jestem specjalistą i sugeruję jeszcze zasięgnąć porady w sprawie tego parametru 05 u fachowców (czasem zdarzają się błędy logiczne, które da się skorygować w dmde czy mhdd).

A na dysku jest bootloader?
Może spróbował byś przywrócić/zainstalować ponownie grub
